Of course, the correct answer to each question is yes. There is certainly a conflict in the institutional hierarchy. Celeste should have informed Dr. Schwartz of the change in her workload. Her decision adversely affected the care she provided. Celeste has put her patient at risk for negative outcomes. She also could have put herself at risk for malpractice, and could have jeopardized both her job and her license.

Did this clinical scenario clarify why it is important that all parties understand to whom the NP ultimately reports? If Celeste had shared her concerns with Dr. Schwartz and Robert immediately, this incident might have been prevented. Further, it would have been best if Celeste’s place in the organizational chart had been put in writing when she was hired.
